Prepare For Your Visit

Before making an appointment, please contact your insurance company to determine if our services are covered under your plan, and if you need a referral from your primary care provider. If your insurance requires a referral and you are unable to get the referral before your visit, we will need to reschedule your appointment.

You will receive new patient paperwork in the mail prior to your appointment.  Please complete and bring the paperwork with you to your appointment to speed up your registration process.  If you prefer, you can also print the forms by visiting our New Patient Forms page.

We ask that you arrive early for your appointment.  The arrival time will depend on the type of appointment, so our staff will inform you of what time you should plan to arrive.

Please bring all insurance cards, a photo ID, and a current list of medications. Inform our medical staff of any known allergies.

It is important that we receive relevant medical records prior to your appointment. If you are self-referring, please contact your healthcare provider and ask that your pertinent records are sent.  If you intend to carry your records with you, please be sure we receive all recent test results and radiology studies, including a disc of your images.

If you are unable to keep your appointment, please notify our office 24 hours before your appointment.

Financial And Billing Policies

At Urology Specialists of Milford, we value the trust and responsibility you place in us to care for you. If you need clarification on our billing policy or you cannot find your answer here, please call our billing and insurance specialist, Sue Lavallee, at (508) 473-6333.

If we participate in your insurance:
“Urology Specialists of Milford will bill your insurance for you and accept the contracted payment rates. We will collect your co-payment, co-insurance, and deductible owed at the time of service.

You are responsible for any amounts the insurance plan deems not covered (a non-covered benefit) up to the entire amount. If we do not receive payment from your insurance provider in a timely manner, we will transfer any remaining balance to your responsibility. We request your assistance in following up with your insurance company to resolve any non-payment issues.

If we do not participate in your insurance:
Urology Specialists of Milford sees patients who have insurance with which we do not participate. In other words, we are then considered an “out-of-network provider” or “non-par provider.” These patients will be fully responsible for total charges. Payment in full will be collected at the time of the visit. We will provide you with an estimate of your patient responsibility and a receipt and documentation so that you can seek reimbursement from your insurance company, if applicable.

If you are a self-pay patient:
If you do not have insurance or you do not have coverage for your office visit procedure or surgery, you will be fully responsible for total charges. You will be expected to pay for the services when you check-in for your office appointment or prior to your surgery. We will provide you with an estimate of your patient responsibility and a receipt so that you can seek reimbursement from your insurance company, if applicable.

If you have a copay, coinsurance, and/or deductible:
Urology Specialists of Milford will collect all copayments, coinsurance, and deductibles due when you check-in at your office visit. If you have Medicare as your primary insurance and you do not have a Secondary policy, you will be responsible for your 20% coinsurance when you check-in for your office visit.

If you have an outstanding patient balance:
Urology Specialists of Milford will collect all outstanding patient balances when you check-in at your office visit or prior to your scheduled surgery. If you have an outstanding insurance claim that has not yet been adjudicated, we reserve the right to estimate your patient responsibility for that visit and collect payment at your next office visit or prior to your surgery.

Credit Card on File Policy:
Due to the high number of deductible plans, and higher patient coinsurance balances, Urology Specialists of Milford requires patients to leave a credit card on file at the time of your visit. We have implemented this billing policy with Instamed, the leading Healthcare Payments Network. We will securely save your encrypted credit or debit card on file to cover any balance due after your insurance benefits have been paid and patient payments have been made. After your insurance company mails an Explanation of Benefits to you and our billing office, we will mail you a statement showing this detailed information and your patient responsibility. If patient balances are not paid within 60 days of the date of service, we will charge this amount to the patient’s credit or debit card on file. Of course, patients can also pay balances in installments over a period of time. We are happy to set up a payment plan for patients if you contact our office.

If patients are unable or unwilling to leave a credit card on file with us, patients will be expected to pay for all services rendered on the date of service. We will provide you with an estimate of your patient responsibility.

Credit cards:
As a convenience, the office accepts Visa, MasterCard, Discover, and American Express credit cards.

Additional fees:
There will be a $25 fee assessed for all checks returned unpaid by banks.

Urology Specialists of Milford, LLC participates with many different insurance plans. If you are unsure of your coverage or benefits, please call your insurance company at the telephone number listed on your benefits card to determine your eligibility in advance of your appointment.

It is a good idea for you to understand your current insurance plan. Some plans require that you get approval from your regular doctor to see a specialist. This is called a referral. If this is how your plan is set up, then you will need to get the referral prior to your scheduled appointment with us. If your insurance company requires a referral form or authorization, it is your responsibility to obtain this from your primary care provider before your appointment at our office.

Please bring your insurance card and driver’s license or other picture ID to every appointment. We will update and/or confirm the accuracy of your insurance information at each office visit or other significant services. In order to process claims on your behalf, we must have your complete insurance information, your employment information, and the person responsible for payment. It is your responsibility to inform us in a timely manner of any changes to your billing information. If an insurance company denies payment for incomplete or wrong information, it is your responsibility to make payment in full.

Prescription Refills

At Urology Specialists of Milford, LLC, we accept calls for prescription refills during regular office hours: 8:30 a.m. to 5 p.m., Monday through Thursday and Friday 8:30 a.m. to 4:00 p.m. Please do not leave prescription refill requests on the answering machine. When you call, please be ready to provide the prescription name, pharmacy name, and pharmacy phone number. Unfortunately, prescriptions cannot be re-filled in the evening or on weekends and holidays.

Of course, you are welcome to call your pharmacy and ask them to fax us your prescription refill request. Our fax number is 508 634 0570.

After-Hours Assistance

At Urology Specialists of Milford, LLC we are committed to providing urgent care when the office is closed.

After-hours services are available for urgent patient issues. To reach the appropriate on-call physician in an urgent situation, please call the office at 508 473 6333 and the answering service will contact the physician on-call. If you are experiencing a medical emergency, please go to the nearest emergency room and call our answering service to contact the physician on-call.
